Form for the registration and participation of a minor athlete. Must be completed and signed by a parent/legal guardian. This document is an integral part of the Event Race Regulations and Terms & Conditions.
I, the undersigned, on behalf of the minor, authorise the registration and participation in the Event and declare that I have read, understood and fully accepted the Race Regulations, the Terms & Conditions, official notices and all operational/safety instructions published on the Event’s official channels. I undertake to ensure the minor complies with all rules and instructions provided by staff, race officials and safety personnel.
I declare that the minor is physically and mentally fit to take part in an open-water and mixed-format sporting event. I undertake to provide the required documentation (including a valid competitive medical certificate and/or membership/registration where applicable) within the stated deadlines. In the event of missing or irregular documentation, I accept that the Organisation may deny participation in accordance with the applicable rules and conditions.
I acknowledge that the Event is physically demanding and involves inherent risks typical of open-water and mixed-format competitions, including: changing weather conditions, water temperature, waves/currents, limited visibility, contact with other competitors, slips/falls and impacts on the beach and during transitions, fatigue, cramps, risk of hypothermia/heat illness and other risks related to sporting activity. I accept such risks and confirm that the minor’s participation is voluntary.
To the extent permitted by applicable law, I hereby release, waive and hold harmless the Organisation (Nuotatori Canavesani A.S.D.) and all persons/entities acting for the Event (directors, staff, volunteers, race officials, stewards, medical and safety personnel, partners, and venue/area managers), from any claim, action or demand connected with injuries, illness or damages suffered by the minor, as well as damage to property/equipment of the minor or third parties, and from loss/theft/damage of personal belongings, except in cases of wilful misconduct or gross negligence by the Organisation as established under applicable law, and without prejudice to any non-waivable rights.
I accept that, for safety reasons or for violations of rules/instructions, the Organisation may stop, suspend or remove the minor from the race. In such cases, unless otherwise stated by published policies or required by law, no refund of the entry fee is due.
I authorise, free of charge and without territorial limits, the recording and use of the minor’s image and voice (photos, videos, live/stream recordings, interviews) produced during the Event and related activities, for informational, documentary and promotional purposes of the Event, on official channels and in third-party media connected to the Event. Use will respect personal dignity and will not be contrary to law.
I declare that I have read the Organisation’s Privacy Notice. Personal data of the minor and the undersigned are processed for organisational, administrative, safety and Event communication purposes, in compliance with Regulation (EU) 2016/679 (GDPR) and applicable laws.
Attach: copy of parent/guardian ID (and, if required by the Organisation, the minor’s ID) + any guardianship/custody document where applicable.
